Why Is AI Often Wrong? Five Common Causes

Understand why AI gives incorrect answers by examining five factors: input, model, context, task scope, and data freshness.

What you'll learn

  • Understand common reasons AI answers are inaccurate.
  • Learn to tell whether the issue lies with the model or with the prompt.

Step-by-step guide

1. Insufficient input information

AI has no access to your internal data. If you give it only one sentence, it will 'guess' your industry and context from its training data. Adding background information typically resolves more than half of all accuracy issues.

2. The model's knowledge cutoff

Most models have a training data cutoff date, so the latest regulations, product versions, or market data may not be included. For time-sensitive content, rely on official documentation — AI output is only a starting point.

3. Task scope exceeds AI's capability

AI performs well on tasks with a 'standard answer.' For tasks requiring on-the-ground experience, multi-party negotiation, or non-public data, it can easily produce answers that sound reasonable but are actually wrong.

4. Context length limits

If you paste too much content at once, the AI may only retain the beginning or the end. Asking questions in smaller segments and confirming understanding at each step is more reliable.

5. Hallucination

Even when everything you provide is correct, AI can still fabricate citations, numbers, or legal clauses. Always verify important information against the original source, and add a 'no fabrication' rule to your prompts.

Good example / Bad example

Good practice

Please answer based only on the contract text I've pasted; if the clause isn't mentioned, respond with 'not covered.'

Bad practice

What does Article 42 of Taiwan's Labor Standards Act say? (accepted without verification)

Common mistakes

  • Directly quoting legal clauses or standard numbers generated by AI.
  • Expecting AI to remember a 20-page document without breaking it into segments.

Cautions

  • Regulatory, financial, and medical information must always be verified against official documentation.
